POSITION: Maternal Child Health/ MCH Coordinator
SALARY: $ 60,000 exempt salaried position

EDUCATION/TRAINING: Registered professional nurse licensed pursuant to Section 12 of the Illinois Nursing Act of 1987 (IL Rev. Stat. 1989, Ch. 111, par 3512) with a bachelor’s degree, preferably BSN, from a recognized or accredited program and one year of experience in community health or maternal and child health nursing. Knowledge of WIC and Family Case Management programs a plus.

  • Directs the planning, organization, development, and evaluation of program activities.
  • Participates in annual grant preparation, including budget, objectives, narrative, and nutrition education plan.
  • Provides direction, supervision, and evaluation of program staff.
  • Assures that staff meet guidelines of USDA, IDPH and all other program agencies.
  • Serves as resource person for in-service education and training program for MCH staff, orients, and trains new staff.
  • Analyzes and evaluates programs to insure and improve the quality of services offered.
  • Monitors and evaluates monthly caseload statistics and implements corrective action as indicated.
  • Assures maintenance of program activities records to meet State and Federal requirements.
  • Assures that monthly reports are submitted to state and regional supervisors as required.
  • Provides a monthly statistical report to administration on caseload management and program hours.
  • Participated in assessment of program activities during state and federal program review/audits.
  • Assures provision of nutrition education as required by the State.
  • Provides follow-up of program decisions by State.
  • Oversees operation and functioning of state-wide program computer systems.
  • Facilitates effective working relationships with a variety of community, governmental, and professional agencies.
  • Oversees maintenance of program inventory.
  • Communicates with Director of Client Services regarding program progress and needs.
  • Performs other duties as assigned or requested.

POSITION: Health Promotion Coordinator
SALARY: Full time exempt salaried position starting at $48,500.00.
EDUCATION REQUIREMENTS: Requires a bachelor’s degree (master’s degree preferred) in public health or related field. Two years full-time experience in community and/or health education required.
DUTIES:
• Plans, organizes, and conducts comprehensive public education.
• Performs comprehensive community health assessment.
• Provides consultative services and technical assistance to department staff and outside agencies.
• Analyzes, evaluates, and recommends changes in health education methods, techniques, and resources.
• Serves as resource for KCHD programs and services.
• Serves as resource for family life education to local teens, preteens, and parents.
• Serves as resource for general health education topics across the lifespan
• Assist with planning and marketing of KCHD sponsored conferences and events.
• Prepares and submits reports, statistics, and data as required.
• Plans and prepares request for proposals for grant funding.
• Performs appropriate emergency response duties as required
• Provides education and outreach to community-organizations, law enforcement, first responders, community members, those struggling with substance use and their families on opioid overdose death prevention strategies.
• Primarily engages organizations who are in high-risk areas within Kankakee, Grundy, Iroquois, and Kendall Counties to offer Opioid Overdose Death Prevention Training.
• Provides Opioid Overdose Prevention Training to interested individuals or agencies.
• Performs other duties as assigned

Education and Training
Bachelor’s degree in public health, community health education, social work, nursing, or another related field. Masters Preferred. Two years of experience in community health or a similar setting required.
Job Requirements
Current Illinois driver’s license. Personal vehicle for use in carrying out required duties. Auto insurance as required by state law. Requires vision adequate to read and analyze printed/written documents. Must have proficient written and oral communication skills to communicate effectively with community stakeholders. Knowledge and data entry efficiency with computers required. Must be able to lift, carry, and transport a minimum of 30 pounds. Must be able to navigate stairs. Manual and finger dexterity required for operation of audiovisual equipment and educational materials. Hearing and speech adequate to answer telephone calls and to communicate with target populations, outside agencies, and staff. Maintains strict confidentiality of client and office information. Demonstrates organizational skills and ability to work independently with general direction.

 POSITION:      Full Time Public Health Nurse, Client Services

SALARY:        Union position $27.27 per hour (35 hour week)

JOB REQUIREMENTS:  A registered professional nurse licensed pursuant to Section 12 of the Illinois Nursing Act of 1987 (Ill. Rev. Stat. 1989, Ch. 111, par. 3512); OR A Bachelor of Science in Nursing (BSN) degree from a recognized or accredited program and one year of experience in community health nursing preferred.  

DUTIES:

  • Implements required procedures used in application of laws, policies, and IDPH/CDC regulations pertaining to communicable disease, foodborne illness, and VFC surveillance. 
  • Performs investigation and surveillance activities of all reportable diseases, and foodborne illness.
  • Responsible for preparation, implementation, and closure of clinic(s) for services (i.e. flu, immunization, STD, TB)
  • Serves as resource and liaison to local healthcare providers, schools, general public and staff on issues pertinent to communicable, reportable and vaccine preventable disease, VFC program; foodborne illnesses; preparedness, travel medicine. 
  • Administers medications and immunizations per standing/doctor’s orders
  • Complies with all VFC rules and regulations regarding vaccine ordering, storage, and handling.
  • Following IDPH guidelines, MD standing or written orders, authorizes laboratory and radiologic screenings appropriately
  • Educates on proper collection, handling, and mailing of mail out specimens for CD/TB/STD to state or other facilities per IDPH guidelines.
  • Maintains organized, accurate, confidential client files
  • Maintains inventory of vaccine(s), medication(s) and supplies needed for clinical services
  • Keeps abreast of changes/trends in the prevention and control of communicable, reportable, vaccine preventable, and chronic diseases
  • Enters all applicable data for surveillance purposes into IDPH/CDC software and uses software to create applicable statistical reports. 
  • Performs appropriate emergency response duties as required
  • Performs other duties as assigned
