Expecting a baby or parenting a newborn under 6 months of age? The Better Birth Outcomes – Comprehensive (BBO-C) program is here to support you every step of the way. This free program pairs Kankakee County residents with an experienced Registered Nurse who will work with you one-on-one—during pregnancy and after your baby is born. Together, we’ll focus on your health, your baby’s well-being, and the support your family needs to thrive.

What is BBO-C?

BBO-C is a statewide program (outside the City of Chicago) developed by the Illinois Department of Human Services (IDHS) Bureau of Maternal and Child Health. It focuses on serving pregnant and postpartum individuals and their infants through comprehensive, one-on-one nursing assessments and personalized support. The goal of BBO-C is to reduce maternal and infant morbidity and mortality, especially in underserved communities.

How It Works

At the heart of BBO-C are Nurse Navigators, who conduct in-person assessments during pregnancy and after birth, provide immediate nursing interventions, and connect families to medical homes and community resources. Supporting families to overcome barriers to care and help them achieve their health goals. This model ensures that every family receives the right care at the right time, from prenatal visits to postpartum follow-ups, all tailored to their unique needs.

BBO-C provides in-person and telephonic services such as:

  • Personalized support and care planning
  • Evidence-based health assessments
  • Health screenings
  • Medical care coordination
  • Linkage to Managed Care Plan benefits and services
  • Health and parenting education to support your baby’s health growth and development Connection with dieticians, lactation consultants and doulas
  • Emotional and Mental Health care and support during and after pregnancy
  • Assistance to address substance use concerns
  • Referrals to healthcare providers, transportation, community partners and more

Eligibility

BBO-C is available to eligible low-income pregnant people and parenting families with infants up to six months of age in Kankakee County who meet risk eligibility guidelines.
